If you have any questions or concerns, need help with the registration process or should need to reschedule your appointment, please do not hesitate to call our Elgin office at (847) 468-9900 or our Lake Zurich office at 847-550-4984.  We request that you kindly give at least 24-hours notice prior to any rescheduled or cancelled appointments.

1. New Patient Registration Form
2. Patient Insurance Form
3. Medical History

Please complete all three of these sections as completely and accurately as possible prior to your appointment.
